Please arrive at least 30 minutes early for you appointment.  If you are a new patient or any of your personal informaton has changed, you will need to download, print, and complete the new patient packet. once completed, either fax it to 713-266-5335 prior to your appointment or email it to Mannortho@gmail.com  You will also need to bring the following documentation and information to your appointment.  

1) Drivers License or goverment picture ID

2) Proof of Insurance

3) Referring physician order (referral if applicable)

4) Any x-rays or test results related to your medical condition.

 Appointments 

Please call the office to make an appointment in advance. Appointment requests are also available on-line. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery. 

Fees & Payments 

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered. We accept cash, checks, MasterCard, and American Express for your convenience. In addition to CareCredit.

Prescriptions & Renewals 

Please request all prescriptions and authorizations for renewals between 9:00am–4:00pm, Monday–Friday, when our full records are available. Renewals requested will be filled or evaluated within 48 hours.   Please direct all renewals directly to your pharmacy.  We are notified electronically from your pharmacy.
